Referrals to ACDP

Who is eligible?

Adults 18 years of age or older who live with:
  • ​aphasia
  • apraxia of speech
  • dysarthria or
  • cognitive communication disabilities due to a stroke, brain injury or brain illness.


Who can refer you?

  • Health care professionals or community agencies
  • Family members, friends, or supporters
  • Self-referral​​
